y - y1 = m(x - x1)
slope(m) = -5
(4,1)...x1 = 4 and y1 = 1
now we sub
y - 1 = -5(x - 4) <===

General point slope form is :

y – y1 = m (x – x1)

Given : 
X1 = 4
Y1 = 1
m = - 5

Substituting the values into the main equation:
We get:
y-1 = -5 (x - 4) 
This is the point slope form.We can simpify further to get the slope intercept form.
Thus,
y -1 = -5x + 20
y = -5x + 20 + 1
y = -5x + 21
