This is because if one constantly uses a ruler which says 2 CM that is actually 1 CM it will create a constant error. However, the other options may create errors in some measurements while not in others.

These are all actually affect the measurement error in an experiment. But of course misreading the instrument ALWAYS affect measurement error. So, the best option will be the D i.e; misreading the instrument.
